On this page:
http://www.philsch.k12.pa.us/cgi-bin/forms/formlist.pl you'll find the EH-36 as a downloadable form (scroll down; it's number 35 in the list), which is what you use to request that your child be allowed to attend a school other than your own (i.e., the one in whose catchment you reside). Be sure to find out from the PSD when the deadline is! For any given schoolyear, the deadline is almost a year earlier - November of the previous year. So do your homework! There are other forms accessible by this page too; not sure (yet) what they do. |

You should be able to pick up the forms at your neighborhood school. They are also available at the main School District office on North Broad St. (below Spring Garden). Go to the Office of Student Placement on the first floor.
The reason you can't print it off the web is due to the fact that it is a 4-part "carbon-like" form. Different pieces go to different parts of the district and one is yours to keep. You can mail your child's EH-36 in. But to be sure it actually ends up in the right place on time, I recommend returning the form directly to the Office of Student Placement and have the person who accepts it sign and date it, so you have proof of timely submission. I would also suggest regular follow-ups after you submit your EH-36. We followed all instructions and met every deadline but were still left wondering in mid-late August where our child would be attending school in September. Finally, after several panicked phone calls to several different levels of people we received confirmation of where he would be attending. This same story wrings true if you are in need of any special services for your child.

One application, but you list up to five schools in order of choice, I believe.

Another tip -- be sure to photocopy the EH-36 and give copies to all the schools you have listed. I know Greenfield often has openings become available after the formal EH-36 process is complete and they refer to the copies of the forms they have received when they are filling those slots. I suspect other schools do the same. Not only is it more convenient for the school, but also it shows you have some level of interest in and commitment to the school.
