The zone number--41 instead of the current 19141 zipcode--and the phone number beginning with the letters DA and no area code--date the dog tag to the early 1960's or before. Zipcodes.com says zipcodes were introduced in 1963.

I checked voter registration records and found nobody named Kovalcyn registered to vote in Pennsylvania today. Nor is anybody by that name locatable by a google search.

I would not be surpised if the tag was ordered at Vahle's pet store around 6th and Olney, the leading pet store in the area in the 1950's and 1960's, where I got tags for my dog then. I frequently went there in the 1960s for food, toys, grooming equipment, leashes and nail clippings for my border collie Frisky.
